Your First Visit

It is helpful if you arrive 15 minutes before your scheduled appointment when you visit our office for the first time. You can complete our office information sheet then or bring it with you already complete. This also allows time to copy your insurance card and take care of any concerns you may have. In addition, it’s best to bring any medications you are currently taking.

We will also give you a copy of our privacy policy at your first visit. Take time to read this document to assure you understand your rights regarding your personal health information.

While it would be difficult to provide a list of all the adult health care areas we serve, and all the tests we offer that support them, we will detail some of them here. As with all health care, our office is always adding new equipment and our staff brings new expertise.

Office Hours
Our office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. To speak with someone during regular hours, call 920-452-6000.

After regular office hours and on weekends, our physicians are available through the answering service at 920-451-7476.

Insurance and Billing Information
We realize that the handling of the financial part of your health care is important to you. Your insurance contract is between you and the insurance company. As a courtesy to you, we will be happy to prepare and submit a claim to your insurance carrier. You are responsible for all charges not covered by your insurance company. If, after a reasonable amount of time, your insurance carrier has not honored your claim, you will be responsible for full payment of the total charges.

We accept Medicare assignment. We also accept Medical Assistant patients.
